How to request account deletion

1. Fill in the form below with your name and the email address you use to sign in to eSSIST. You do not need to install or open the app.
2. Or send an email to privacyweb@itracHEALTH.com (or info@itracHEALTH.com) with the subject “Account deletion request” and the email address of your account.
3. We will confirm the request with you by email before deleting the account, to make sure the request comes from the account owner.
4. We will email you when the deletion is complete.

What data is deleted

• Your account profile (name, email address, phone number) and login credentials.
• Your app settings and preferences.
• Care-circle connections linked to your account.

What data may be kept, and for how long

Account profile and login data are deleted within 30 days. Some records may be kept longer where the law requires (for example, health records created as part of care provided by your provider, and security and audit logs), and are then deleted.
